   * What led up to the situation?
I run xdm and sometimes I get error 256 as an exit code.
It also doesn't delete the lock file /var/run/xdm.pid and can't start after the next reboot.
For me it seems xdm doesn't exit properly. It happended many times, but not every time xdm is being used.

   * What exactly did you do (or not do) that was effective (or
Deleting /var/run/xdm.pid after the reboot if xdm can't start helps.
