Not sure what causes this, but there is still an empty/generic desktop icon
displayed for Lazarus (4.0) in GNOME instead of the correct one. I have
checked the installed desktop icons and the desktop file and everything
looks correct.

Yes, I have. I am using a standard desktop Debian Trixie installation with
GNOME.

Yes. It needs to be installed.
It's absence might be a problem, but clearly not the issue here.

Unfortunately still the same problem with Lazarus 4.8 on latest Debian
Forky. (Lazarus 4.6 does not seem to be available for some reason.)
