   * What led up to the situation?

After not upgrading packages in over 10 days, I found that sometimes
video playback locked up, windows failed to refresh unless I switched to
vt1 and back to vt7 and in fact that I could not restart Xorg (using lxdm)
without rebooting the machine.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?
   * What was the outcome of this action?
   * What outcome did you expect instead?

We have to be careful not to conflate multiple issues here. There are at
least two issues:

Video playback hangs the GPU. Most likely a Mesa issue.

Xorg cannot be restarted after that (you reported this upstream at
https://bugs.freedesktop.org/show_bug.cgi?id=99138), presumably a radeon
kernel driver issue.
